A disclosure must be made in writing by filling out the Disclosure of Wrongdoing Form.
Please contact our office to get a copy of this form, then submit it using secure file transfer or deliver it in person. Do not email the form as this is not a secure method of communication.

An employee who seeks advice about making a disclosure, made a disclosure, co-operated in an investigation, or declined to participate in a wrongdoing must not be reprised against or be directed that reprisal be taken against them. If they suspect a reprisal, they can contact our office.
